Hub Appointments With GP's & Other Health Care Professionals


If you're a patient with a chronic, complex or long term condition then in order to ensure your continuity of care it is important that you are seen by your GP here at the surgery.However, patients with non-chronic problems may get an appointment with a GP or other health care professional far more quickly or at short notice at one of the community hubs.


Monday to Friday 6.30pm - 8.00pm at Parkview Medical Practice


Saturday 8.00am - 12.00 at St Mary's Health Centre


Sunday 9.00am to 12.00am at Briarwood Medical Centre


The service offered is for pre-booked appointments and also a limited amount of appointments which are bookable on the day.Please note that once hub availability is full, you may be directed to an alternative service.


In a genuine emergency you should call 999.  Chest pains and/or shortness of breath constitute an emergency.

If you require a routine OR urgent blood test, please see the following information to support you in accessing the most appropriate service.

Local NHS staff are working extremely hard to maintain services during this difficult time and the continued cooperation of patients is greatly appreciated.

Only the patient requiring the blood test should enter the clinic unless assistance is required by a carer or relative for the duration.
Booking routine blood tests
Routine blood tests can be booked in the community or at your local hospital.
Routine blood clinic option 1: To make an appointment at a community based blood clinic: 

  • If you live in Chorley and South Ribble, appointments can be made by contacting your GP practice directly.
  • If you live in Preston, please visit: https://www.lscft.nhs.uk/Phlebotomy to access the contact details for your local community clinic.

Routine blood clinic option 2: Appointments can be made at Royal Preston Hospital or Chorley and South Ribble District General Hospital, operating Monday to Friday from 9am until 5pm.
To book your appointment, please call 01257 247095.
You should only attend one of these hospital clinics for a blood test if:

  • your doctor has instructed you to do so, AND
  • has supplied you with a blood form to bring with you to your appointment.

The blood form is a requirement for your appointment so please remember to bring it along. If you have not received a blood form, but have been instructed to attend a hospital clinic, please contact your GP practice directly before attending.
 
If you are attending Royal Preston Hospital for a routine blood-test, please arrive at the main entrance and turn right towards the ambulance desk; the clinic details will be sign posted from here.
 
If you are attending Chorley and South Ribble District General Hospital for a routine blood-test, please arrive at the front door of the Urgent Care Centre.
 
Booking urgent blood tests
 
There are a number of ways to access urgent blood test clinic appointments. Both clinic options provide the same service. Please note the different opening times, contact details and location with the hospital sites.
 
You should only attend one of these hospital clinics for an urgent blood test if:

  • your doctor has instructed you to do so, AND
  • has supplied you with a blood form to bring with you to your appointment.

The blood form is a requirement for your appointment so please remember to bring it along. If you have not received a blood form, but have been instructed to attend a hospital clinic, please contact your GP practice directly before attending.
 
Urgent clinic booking option 1: Clinics at Royal Preston Hospital or Chorley and South Ribble District General Hospital site, operating Monday to Friday from 9am until 5pm.

These urgent clinics will take place in the usual blood clinic locations at both Royal Preston Hospital and Chorley and South Ribble District General Hospital.
